A partnership between the Franklin County Board of Commissioners and Franklin County Public Health, Health Works Franklin County is a “health in all policies” (HiAP) initiative. The goal is to integrate and coordinate social determinants of health data, policy and planning efforts with the public health system. Led by a voluntary county Steering Committee, Health Works Franklin County is made up of cross-sector teams to implement and coordinate data sharing, planning and policy strategies. Health Works Franklin County will use a collective impact framework to incorporate the multiple elements of the HiAP approach emphasizing prevention, well-being and health equity.

To track the health impact of its work over the short and long-term, Health Works Franklin County will adopt a common reporting platform to identify, track and align data, agency strategic planning efforts and community-based initiatives and resources. The site will integrate and display data identified at the county and zip code levels for communities and agencies.

As outlined in Figure 1, Health Works Franklin County will collaborate with existing community engagement, clinical care integration and issue-specific initiatives already occurring across the county. Health Works Franklin County is meant to be an opportunity to build on existing efforts creating an upstream perspective to address those conditions and barriers related to health equity.

Working in collaboration with existing community based, clinical and health-specific initiatives, Health Works Franklin County is well positioned to advance innovative efforts that systematically address the community conditions that impact health. The cross-sector teams of Health Works Franklin County will include government agencies, non-profit organizations and healthcare organizations.
